They may be downloaded, printed, and used to make copies. If you do not have Adobe Acrobat Reader, you can download and install a copy from Adobe's website.

Anyone who is 18 or older can receive the following immunizations at the Cerro Gordo County Department of Public Health Immunization Clinic:

Hepatitis A: Two (2) doses of the vaccine, given at least six (6) months apart are needed for lasting protection.

Hepatitis B: Full immunization requires three (3) doses of vaccine over a six (6)-month period. Usually the first two doses are given one (1) month apart, and the third dose, five (5) months after the second.

Varicella (Chickenpox): Two (2) doses of the vaccine four to eight weeks apart are needed for anyone over 13 years of age.

MMR: (measles, mumps, rubella).

Pneumonia vaccine: Recommend one dose after age 45, and one dose after age 65.

Influenza vaccine (when available): Recommend annual immunization.

Meningococcal vaccine (meningitis)

Shingles vaccine

Td (tetanus): Recommend one dose every ten (10) years.

Tdap Combination tetanus booster and pertussis vaccine.
